Open Enrollment For Current Members
Annual Enrollment for 2025 will run from Session 1: October 11 to November 1 or Session 2: October 25 to November 15.
Currently enrolled employees (plan members) will receive an Annual Enrollment letter in a green envelope from the Medical Trust approximately one week before their Annual Enrollment period. This letter will include information about how to access the enrollment site. During annual enrollment all employees are required to log into MyCPG and select their health benefit choice. If this is not done by the employee, then the Parish Administrator is required to enroll each employee. If this is not done for dental then they will have no dental coverage for 2025.
If an employee takes no action, and their current medical plan is offered for 2025, their medical plan selections will automatically carry over to 2025, and any applicable rate increases will apply.
Employees can also download the Sydney Health app on a cell phone and will be able to view insurance cards, deductibles, virtual doctor appointments, claims, and more great info.
IMPORTANT REMINDER: Members will make their plan selections on MyCPG Accounts using the email address and password associated with their MyCPG Account. The Client ID number is no longer used to access the account. It is important for all members to create an account prior to Annual Enrollment if they have not already done so.
For assistance, employees may contact CPG Client Services at (800) 480-9967, Monday to Friday, 8:30 AM to 8:00 PM ET, or email mtcustserv@cpg.org.
Health Benefits Enrollment
For Health Benefits Administrators and Newly Eligible Individuals
Important Note on Newly Eligible Individuals Enrollment
There is no waiting period for newly eligible individuals. Newly eligible individuals have a period of 30 days immediately following the hire date or date the individual became part of the Participating Group or became an eligible individual to elect a health, dental, group life, and/or disability plan for the remainder of the current plan year. Plan elections, once made, cannot be changed for the remainder of the current plan year unless the member experiences a Significant Life Event or HIPAA Special Enrollment Event.
If you are hiring a new employee, please complete the appropriate enrollment forms, enter new hire enrollment in MAP, and email completed forms to Jessica Lee (jlee@edola.org) at the diocesan office as soon as possible to avoid possible denial of coverage.
